CNA License must be from Florida. If the applicant has an out-of-state CNA License, they can apply for a Multi-State Floridian CNA License. We must be able to verify CNA Licenses on the Florida Department of Health website.
Tuberculosis (TB) Certification
Please present one of the following:
PPD (completed at least within the last two years)
QuantiFERON (Completed within the last two years)
Chest X-Ray (Only accept if dated between 2019-2024)
Trainings
Please present the following:
Valid BLS CPR (Must have online verification ability, EX-QR code)
Alzheimer's CEU with no less than 2 credit hours
HIV/AIDS CEU with no less than 2 credit hours
Assisting with Self Medication CEU
Automobile Insurance Card
Current auto insurance card must have Applicants name printed on it OR the auto insurance policy must be provided stating that Applicant is a driver for the car (declaration page).
AHCA Background Screening
Also known as “Level 2 Fingerprinting”. If applicant does not have AHCA BGS, they can request assistance via HR team to set an appointment. Applicant must pay out of pocket for AHCA BGS.
